Patients in Group A (n = 15) were treated with TSPLS, and those in Group B (n = 24) were treated with traditional laparoscopy. Both groups underwent CSP resection and lower uterine segment repair via plastic surgery. Based on their condition and intraoperative bleeding, the patients were treated with uterine artery preligation. The operative time, preoperative pretreatment, intraoperative blood loss, decrease in beta-human chorionic gonadotropin (β-hCG) level, postoperative vaginal bleeding time, postoperative intestinal ventilation time, length of hospital stays, complications, and scar cosmesis assessment were compared.</p><p><strong>Results: </strong>The scar cosmesis score was significantly lower in Group A than in Group B (p < 0.01). No significant differences in intraoperative blood loss, absolute decrease in β-hCG level, absolute decrease in blood hemoglobin level, absolute increase in blood C-reactive protein level, postoperative vaginal bleeding time, postoperative intestinal ventilation time, length of hospital stay, and complications were observed between the two groups (p > 0.05). Abstract

目的:探讨经脐单孔腹腔镜(TSPLS)与传统腹腔镜治疗II型、III型剖宫产瘢痕妊娠(CSP)的临床及美容效果差异。方法:回顾性分析2018年6月至2021年6月湖北省妇幼保健院收治的II型和III型CSP患者39例,根据患者意愿分为A组和B组。A组(n = 15)采用TSPLS治疗,B组(n = 24)采用传统腹腔镜治疗。两组均行CSP切除及下段整形修复术。根据患者的情况及术中出血情况,对患者行子宫动脉结扎术。比较两组手术时间、术前预处理、术中出血量、β-人绒毛膜促性腺激素(β-hCG)水平下降、术后阴道出血时间、术后肠道通气时间、住院时间、并发症及疤痕美容评价。结果:A组瘢痕美容评分明显低于B组(p < 0.01)。两组患者术中出血量、β-hCG水平绝对下降、血红蛋白水平绝对下降、血c反应蛋白水平绝对升高、术后阴道出血时间、术后肠道通气时间、住院时间、并发症等差异均无统计学意义(p < 0.05)。实验组手术时间略长于对照组(p < 0.05)。结论:TSPLS与传统多口腹腔镜相比具有更好的美容效果,且无手术效果和术后恢复差异,具有临床应用价值。
