We present a case of a 65-year-old male with a ruptured 4.6 cm x 7.6 cm x 5.0 cm distal PFA aneurysm alongside proximal PFA and CFA aneurysms. Endovascular coil embolization successfully treated the aneurysm, with concurrent thigh fasciotomy for compartment syndrome. The patient's complex medical history influenced treatment selection. This underscores the importance of early recognition and tailored management, advocating for endovascular interventions in high-risk patients.
