Les CPTS, une opportunité pour renforcer le lien ville-hôpital

Abstract

Les Communautés professionnelles territoriales de santé (CPTS) sont des structures d’exercice coordonné à l’échelle d’un territoire. L’un de leurs objectifs majeurs est d’améliorer l’organisation des parcours de soins. À travers trois exemples, cet article met en lumière comment les CPTS représentent une opportunité pour renforcer la collaboration entre la Ville et l’Hôpital.

The Communautés professionnelles territoriales de santé (CPTS) are structures of collaborative practice within a specific region. One of their major goals is to improve the organization of care pathways. Through three examples, this article highlights how CPTS offer an opportunity to strengthen the collaboration between the primary care and the hospital care.
