Évaluation du dispositif Medpack® en médecine d’urgence préhospitalière

Abstract

Introduction

Le Medpack®, station de travail primée par le concours Lépine en 2018, a été créé dans le but d’améliorer les pratiques infirmières en termes d’hygiène et d’ergonomie dans le cadre de l’urgence préhospitalière. Avec une nomenclature adaptée, il vise à assurer à lui seul la majeure partie des prises en charge, allégeant ainsi la dotation du sac d’intervention.

Méthode

Nous avons réalisé une étude prospective observationnelle monocentrique menée du 23 juin 2018 au 1^er mars 2019 au sein des six ambulances de réanimation de la brigade de sapeurs-pompiers de Paris. Un cahier de recueil des données auto-administrées était rempli par les infirmiers après chaque intervention. Il concernait l’appréciation de l’hygiène et de l’ergonomie des soins avec le Medpack®, ainsi que la liste des dispositifs médicaux et médicaments utilisés dans le sac d’intervention, en plus de ceux utilisés dans le Medpack®.

Résultats

Soixante-douze pour cent des infirmiers estiment que leurs soins sont plus conformes aux bonnes pratiques d’hygiène, et 76 % perçoivent un gain ergonomique lors de l’utilisation du Medpack®. Celui-ci a été utilisé dans 69 % des interventions avec pose d’accès vasculaire (Apav), le sac d’intervention, quant à lui, s’est avéré inutile dans 75 % des interventions. Lorsqu’il a été utilisé, ce fut à 34 % pour le matériel d’intubation et à 15 % pour le matériel de biologie embarqué. Quarante-huit pour cent du matériel du sac d’intervention a été utilisé dans moins de 1 % des interventions.

Discussion

Le Medpack® permet une pratique des soins paramédicaux d’urgence préhospitalière plus conforme aux exigences d’hygiène, avec une ergonomie plus adaptée à ce contexte. Une nomenclature ajustée du Medpack® permet d’assurer une grande partie des interventions et de repenser la composition du sac d’intervention. Un temps d’adaptation est nécessaire à la modification des pratiques infirmières afin de permettre son acceptation et son appropriation. De plus, sa nomenclature est modulable, c’est pourquoi il pourrait s’adapter à d’autres domaines que l’urgence préhospitalière comme l’exercice libéral ou la médecine de catastrophe.

Introduction

The Medpack® workstation, which won the Lépine competition in 2018, was created with the aim of improving nursing practices in terms of hygiene and ergonomics in pre-hospital emergency care. With an adapted nomenclature, it aims to provide the majority of care by itself, thus lightening the intervention bag.

Method

We carried out a monocentric observational prospective study from 23/06/2018 to 01/03/2019 in the 6 resuscitation ambulances of the Paris Fire Brigade. A self-administered data collection notebook was filled in by the nurses after each intervention. It included an assessment of the hygiene and ergonomics of care with Medpack®, as well as a list of the medical devices and medicines used in the intervention bag, in addition to those used in Medpack®.

Results

Seventy-two percent of the nurses felt that their care was more in line with good hygiene practices, with 76% perceiving an ergonomic gain when using Medpack®. The Medpack® was used in 69% of Vascular Access Placement (Vapp) procedures, while the intervention bag was useless in 75% of procedures. When the bag was used, it was used in 34% of cases for intubation equipment and in 15% of cases for onboard biology equipment. Forty-eight percent of the materials in the intervention bag were used in less than 1% of the interventions.

Discussion

The Medpack® enables pre-hospital emergency paramedical care to be carried out in a way that is more in line with hygiene requirements, with ergonomics more suited to this context. An adjusted nomenclature of Medpack® makes it possible to carry out a large proportion of interventions and to rethink the materials making up the intervention bag. A period of adaptation to its use is necessary to modify nursing practices in order to allow its acceptance and appropriation. Moreover, its nomenclature is modular, which is why it could be adapted to fields other than pre-hospital emergency care, such as private practice or disaster medicine.
