The Unified Modeling Language (UML) and interface design were used in the development of this system. This is a web-based application and was developed using the PHP programming language, HTML, CSS, Javascript, and the MySQL database. The application was developed using the waterfall method, with the documentation method used for monitoring and the black box method for testing. The result of this study is a web application.
