This study aimed to describe the nurse's hand hygiene compliance, identify the factors that influence it and determine solutions to improve it. This study was descriptive-observational. The population of this study was all dialysis nurses in the Dialysis Unit of Dr. Soetomo General Hospital Surabaya. The sample size based on WHO requirements was 200 opportunities for hand hygiene. Identify the root cause using a fishbone diagram—determine alternative solutions to problems using Capability, Accessibility, Readiness, and Leverage methods. Based on the observations, nurses' compliance with hand hygiene was 28.5%. The lowest nurse compliance rate is at the indication before and after touching the patient. The factors that most influence the hand hygiene of dialysis nurses are the lack of knowledge and awareness of the importance of hand hygiene and the performance of the IPCLN team, which is not optimal.
