Establishing Patient Partners: The Health Care Home Model

Abstract

INTRODUCTION Development of the Pinnacle Health Care Home (HCH) model for primary care in New Zealand includes design that enables and promotes patients as active partners in their health care rather than passive recipients. A conceptual framework that underpins the model design has been described as “Patients, families, their representatives, and health professionals working in active partnership at various levels across the healthcare system—direct care, organizational design and governance, and policy making—to improve health and healthcare”.
