Based on the interview results of 175 respondents, 93% said the accident was caused by the negligence of the supervision or personal responsibility, 21% said that it was caused by the absence of carbon gas test since there were no tools for detecting the toxic gas (gas detector) and 78% claimed that theyunderstand that the occupational accidents happened in enclosed spaces due to the negligence and procedural error (SOP). Abstract
