The level of the coronary ostia may vary. In the cases hitherto described, coronary ostia may be located at the level of the sinotubular junction [1], higher [2] or lower [3] the junctional zone. Different types of anatomical variations of the coronary ostia are known as the ectopic coronary artery. When the origin of the coronary artery is located more than 1 cm above the sinotubular junction the term “high takeoff” is used [4]. The high position of the ostium of the right coronary artery above the right sinus of Valsalva is rare. Nevertheless, the studies show that the high takeoff is more typical of the RCA than the left coronary artery. The position of the coronary ostium in the tubular part of the ascending aorta decreases coronary perfusion.
