Vascular structure and histological morphology changes in GDM placentas were examined to generate basic experimental data for the diagnosis and prognostic determination of GDM.</p><p><strong>Methods: </strong>This case-control study involving 60 placentas, 30 from healthy controls and 30 from patients with GDM. Differences in size, weight, volume, umbilical cord diameter, and gestational age were assessed. Histological changes in the placentas in the two groups were analyzed and compared. A placental vessel casting model was constructed using a self-setting dental powder technique, to compare the two groups. The placental cast microvessels of the two groups were compared using scanning electron microscopy.</p><p><strong>Results: </strong>There were no significant differences in maternal age or gestational age between the GDM group and the control group (<i>p</i> > .05). The size, weight, volume, and thickness of the placentas in the GDM group were significantly greater than those in the control group, as was umbilical cord diameter (<i>p</i> < .05). Immature villus, fibrinoid necrosis, calcification, and vascular thrombosis were significantly greater in the placental mass in the GDM group (<i>p</i> < .05). 摘要

目的:通过血管铸型技术、电镜及病理检测技术观察妊娠期糖尿病(GDM)胎盘与正常胎盘微血管形态及病理变化。检测GDM胎盘血管结构和组织学形态变化,为GDM的诊断和预后判断提供基础实验数据。方法:本病例-对照研究纳入60个胎盘,其中30个来自健康对照,30个来自GDM患者。评估大小、体重、体积、脐带直径和胎龄的差异。分析比较两组胎盘的组织学变化。采用自凝牙粉技术建立胎盘血管铸造模型,对两组进行比较。采用扫描电镜对两组胎盘铸型微血管进行比较。结果:GDM组与对照组产妇年龄、胎龄差异均无统计学意义(p > 0.05)。GDM组胎盘大小、重量、体积、厚度及脐带直径均显著大于对照组(p < 0.05)。GDM组胎盘团块未成熟绒毛、纤维蛋白样坏死、钙化、血管血栓形成明显增多(p < 0.05)。糖尿病胎盘铸型微血管末端分支稀疏,末端明显减少,绒毛体积明显减小(p < 0.05)。结论:妊娠期糖尿病可引起胎盘的大体和组织学改变,尤其是胎盘微血管的改变。
