Une cellulite nécrosante cervicale révélant un syndrome de Lemierre

Introduction

Le syndrome de Lemierre est caractérisé par une thrombophlébite septique de la veine jugulaire interne (VJI) consécutive le plus souvent à une infection oropharyngée.

Cas clinique

Nous rapportons un cas de syndrome de Lemierre survenu dans un contexte d’angine et de cellulite nécrosante cervicale chez un patient de 45 ans. L’écho-doppler des vaisseaux du cou et la tomodensitométrie cervico-faciale ont mis en évidence une thrombophlébite de la VJI. Aucun germe n’a pu être isolé dans les prélèvements effectués (hémocultures, pus). Le traitement a été médico-chirurgical associant une antibiothérapie, une héparinothérapie et un débridement chirurgical des tissus nécrosés avec extraction du thrombus après ligature et section de la VJI. Les suites ont été simples.

Discussion

Le syndrome de Lemierre est une pathologie rare mais grave. Sa faible incidence fait de lui une pathologie oubliée. Il doit être systématiquement évoqué devant toute infection oropharyngée avec présence d’une importante tuméfaction douloureuse cervicale.

Lemierre syndrome is characterized by a septic thrombophlebitis of the internal jugular vein (IJV) following an oropharyngeal infection.

Case report

We report a case of Lemierre syndrome that occurred in a context of angina and necrotizing cellulitis of the neck in a 45-year-old patient. The Doppler ultrasound exam of the neck vessels and a neck CT showed an IJV thrombophlebitis. No germ could be isolated in the samples (blood culture, pus). The treatment associated antibiotics, heparin and surgical debridement of the necrotic tissues with extraction of the thrombus after ligation and section of the IJV. The postoperative course was uneventful.

Discussion

Lemierre syndrome is a rare but serious disease. Its low incidence makes him a forgotten disease. It should be systematically suspected in any oropharyngeal infection with the presence of a large painful swelling of the neck.
