The diameter of mass was > 20 mm on the images, and a tumor thrombus-like structure was identified in the caudal vena cava. We diagnosed the adrenal grand mass as a benign tumor using endocrinologic and two contrast-enhanced computed tomography examinations. Surgical resection was performed, considering the risk of complications due to the mass in the caudal vena cava. Histo-pathological examinations of the surgical specimen revealed that the adrenal grand mass and the mass in the caudal vena cava were lymphocytic adrenal medullitis and thrombosis, respectively. Neither of the masses showed neoplastic changes. After surgery, although a small clot was found in the caudal vena cava, the patient had a good outcome and was asymptomatic.
