The clinical sequelae of these deformities are the syndrome of heart failure, arrhythmias and stroke. Dusek first described the postnatal persistence of spongy myocardium in 1975 pathologically, but Engberding and Bender made the first clinical recognition with two-dimensional (2D) echocardiography in 1984 [2,3]. Three decades later, with only morphologic assessment available and no definitive genetic pathway, isolated left ventricular noncompaction (LVNC) remains a diagnostic and management challenge.
