It includes a review of outcome measurement and other relevant literature, the service goals and outcomes, and a data plan for demonstrating and communicating the outcomes. This article is the third in a series of three articles that together make up an entire project planning and management document that is based on Rhea Rubin's (2006) Outcome Measurement model.
