The present case report reveals the endodontic management of the four-rooted right maxillary second molar diagnosed with the help of cone-beam computed tomography (CBCT) imaging. The CBCT findings confirmed the presence of four roots (mesiobuccal, distobuccal, distopalatal, and mesiopalatal) and four root canals.
