Type IV hypoplasia, also known as floating thumb, includes two small phalanges attached to the hand with a thin skin bridge containing neurovascular bundle. A fifteen days old boy had a mass-like floating thumb, unlikely to frequently encountered. The family was advised to seek genetic counseling for accompanying abnormalities of the baby. Six days later, the baby showed up with the congested thumb.
