摘要

目的探讨超声造影对类风湿关节炎滑膜炎活动性临床缓解期的预测价值。方法选取2017年9月至2019年7月内蒙古人民医院诊断的类风湿关节炎缓解期患者74例(148个腕关节)。选取滑膜增厚的腕关节,采用功率多普勒超声和超声增强检查滑膜内微血管。比较两种方法在滑膜厚度、血流信号显示率、血流分类等方面的检查结果。结果①超声造影检查滑膜厚度平均值为(5.17±2.06)mm,优于功率多普勒超声检查滑膜厚度平均值(4.89±2.00)mm (t=8.765, P<0.001);②超声造影血流信号显示率为59.5%(69/116),优于功率多普勒超声造影血流信号显示率26.7% (31/116)(P<0.001)。③超声造影对血流信号的分类优于功率多普勒超声(Kappa=0.173, P=0.043)。结论超声造影能更准确地反映类风湿关节炎滑膜炎在临床缓解期的活跃性,指导临床治疗。关键词:关节炎;类风湿;临床缓解期;超声造影;功率多普勒超声;滑膜炎症
