Methods: In a clinical trial with a pretest-posttest and follow-up design, 45 of 135 breast cancer patients in Amol city in 2017 were enrolled after controlling for inclusion/exclusion criteria. Subjects were randomly divided into 3 groups: reality therapy, neurofeedback therapy, and control. All participants completed the Beck Depression Inventory (BDI) and the State-Trait Anxiety Inventory (STAI) at 3 time points. Data were analyzed using a MANCOVA test. Results: Significant differences were observed in the mean DPI and STAI scores among the three groups at posttreatment. Lower mean posttest depression scores were obtained for the two therapy groups compared with the control group, with better results for the reality therapy (p<0.05). The mean STAI score was significantly lower for the reality therapy group than for the control group (p<0.05), while only a positive trend was observed for the neurofeedback. In sum, a significant improvement of 2.28 to 6.64 points relative to baseline was observed in the mean depression and anxiety scores after the psychological therapies. These therapeutic effects even increased over the 4 months of follow-up, indicating the consistency of therapeutic effects for both treatments (p<0.05). Conclusion: The reality therapy and neurofeedback therapy could effectively decrease state-trait anxiety and depression symptoms in breast cancer patients, with robust effects for reality therapy, and are recommended to these patients. 摘要

前言:本研究的目的是比较神经反馈疗法和现实疗法在改善乳腺癌女性抑郁和状态-特质焦虑心理症状方面的效果。方法:采用前测后测和随访设计的临床试验,在控制纳入/排除标准后,从2017年Amol市135例乳腺癌患者中选取45例入组。受试者随机分为现实治疗组、神经反馈治疗组和对照组。所有参与者在3个时间点完成贝克抑郁量表(BDI)和状态-特质焦虑量表(STAI)。数据分析采用MANCOVA检验。结果:三组患者治疗后DPI、STAI评分均有显著性差异。治疗组和治疗组的抑郁后测平均分均低于对照组,现实治疗组的效果优于对照组(p<0.05)。现实治疗组的平均STAI评分显著低于对照组(p<0.05),而神经反馈组只有阳性趋势。综上所述,心理治疗后抑郁和焦虑平均分较基线显著提高2.28 ~ 6.64分。这些治疗效果甚至在4个月的随访中有所增加,表明两种治疗效果的一致性(p<0.05)。结论:现实疗法和神经反馈疗法能有效减轻乳腺癌患者状态-特质焦虑和抑郁症状,现实疗法效果稳健,值得推荐。考虑到现实疗法的经济性和可获得性,我们推荐现实疗法作为乳腺癌患者的辅助治疗。
