Our aim is to compare the patients with benign or malignant lesions of the pancreas who underwent laparoscopic distal pancreatectomy (LDP) versus open distal pancreatectomy (ODP), in terms of clinical, pathological and postoperative outcomes. METHODS : Data of patients who underwent distal pancreatectomy for diseases of the body or tail of the pancreas between March 2014 and September 2020 were retrospectively analyzed. The patients were divided into two groups as laparoscopic or open, according to their surgical methods and the groups were compared in terms of demographic characteristics, surgical outcomes, postoperative results and pathological outcomes. RESULTS : There were 40 patients in the LDP group and 43 patients in the ODP group. There were no statistical differences between the two groups in terms of age, sex, body mass index and ASA scores. The mean operative time was 259±71 minutes in LDP and 260±68 minutes in ODP (p=0.86). The pancreatic stump was closed with stapler in 18 (45%) patients in the LDP group and in 6 (14%) patients in the ODP group (p=0.03). The length of hospital stay was significantly shorter in the LDP group (LDP;7.8±1.9 vs ODP;10.9±4.2, p=0.01). Clinically relevant Grade B postoperative pancreatic fistula was detected in 5 (12.5%) patients in the LDP group and 7 (16.2%) in the ODP group (p>0.05). There were no statistically significant differences between the groups in terms of tumor size, the mean number of harvested lymph nodes and the mean number of pathological lymph nodes. 导读:由于技术的进步,在胰腺手术中首选微创手术的比率正在增加。我们的目的是比较行腹腔镜远端胰腺切除术(LDP)与开放式远端胰腺切除术(ODP)的胰腺良性或恶性病变患者的临床、病理和术后结果。方法:回顾性分析2014年3月至2020年9月期间因胰腺体或胰腺尾部疾病行远端胰腺切除术的患者资料。根据手术方式将患者分为腹腔镜组和开腹组,比较两组患者的人口学特征、手术结果、术后结果和病理结果。结果:LDP组40例,ODP组43例。两组在年龄、性别、体重指数和ASA评分方面没有统计学差异。LDP组平均手术时间259±71分钟,ODP组平均手术时间260±68分钟(p=0.86)。LDP组18例(45%)和ODP组6例(14%)采用订书机缝合胰腺残端(p=0.03)。LDP组住院时间明显缩短(LDP;7.8±1.9 vs ODP;10.9±4.2,p=0.01)。LDP组5例(12.5%)、ODP组7例(16.2%)出现临床相关的术后B级胰瘘(p>0.05)。两组肿瘤大小、平均淋巴结清扫数、病理淋巴结平均清扫数比较,差异均无统计学意义。讨论与结论:我们认为对于胰腺良性或恶性病变的远端胰腺切除术患者,腹腔镜方法是安全的首选。
