摘要

目的比较特发性黄斑外膜(IEMM)剥脱和未剥脱的患者手术后的结构形态和视觉功能变化。方法回顾性分析83例(84眼)IEMM患者，根据治疗方法的不同分为两组，观察组:EMM剥离联合ilm剥离;对照组:单一EMM剥离。随访2 ~ 12个月，观察黄斑外膜剥离、黄斑水肿、视力变化，以及视力与中央凹厚度的变化进行线性相关分析。结果两组术后视力明显改善(P <0.05)，视网膜前膜剥离成功。观察组:40眼40例黄斑水肿复发一次(2.5%);对照组:43眼44眼5例黄斑水肿复发一次(11.4%)。两组水肿复发率比较，差异均有统计学意义(P <0.05)，术后边缘视力改善与边缘中央凹厚度改善呈正相关(观察组，r=0.616,P=0.00;对照组，r=0.462,P=0.01)。结论玻璃体切除术联合内限制膜剥离可显著提高手术成功率，减少水肿复发率，改善视功能，改善视力较好，视力的改变与中央黄斑厚度呈正相关。关键词:特发性黄斑视网膜前膜;23-gaugevitrectomy;内限膜剥离;水肿复发
