摘要

目的探讨儿童不同类型幽门螺杆菌(HP)与胃十二指肠疾病类型及胃黏膜炎症严重程度的关系。方法采用western blot法检测112例hp相关性慢性胃炎(CG)和消化性溃疡(PU)患者血清中CagA和VacA的抗体。观察胃黏膜病理变化。结果112例CagA、VacA抗体检出率分别为82.14%和82.14%,差异无统计学意义(P < 0.05)。慢性胃炎(CG)患者CagA、VacA抗体检出率分别为79.10%和83.58%,消化性溃疡(PU)患者CagA、VacA抗体检出率分别为86.67%和91.11%,两组患者CagA、VacA抗体检出率差异无统计学意义(P < 0.05)。112例儿童Hp感染中,I型Hp占80.36%,Ⅱ型Hp占11.61%,中间型Hp占8.03%,三种Hp在CG和PU的检出率差异无统计学意义(P < 0.05);90例Ⅰ型HP患者中,92.22%的患者出现重度胃黏膜炎症,7.78%的患者出现轻度炎症,13例Ⅱ型HP患者中有23.08%出现重度胃黏膜炎症,9例中度HP患者中有55.56%出现重度胃黏膜炎症,差异有统计学意义(P<0.01)。结论CagA和VaCA与CG和PU发病密切相关,但不能作为判断CG和PU发病的依据,当儿童感染Ⅰ型HP时,其胃黏膜损伤最大,应积极治疗。关键词:幽门螺杆菌;儿童;细胞毒素相关蛋白;液泡毒素蛋白;抗体;慢性胃炎;溃疡
