Metastatic disease of the pancreas usually presents initially with spread to the surgical bed, liver, and lungs, and further metastatic disease to the bone is rare. Here, we present a case of pancreatic cancer recurrence with isolated osteoblastic and osteolytic bone metastasis without visceral organ spread in a patient with early stage resected pancreatic head adenocarcinoma.
