Bai","doi":"10.3760/CMA.J.ISSN.1008-6706.2020.06.014","DOIUrl":null,"url":null,"abstract":"Objective \nTo explore the related factors of anterior cruciate ligament rupture with meniscus injury and the effect of arthroscopic surgery. \n \n \nMethods \nFrom July 2014 to May 2018, 98 patients with anterior cruciate ligament rupture admitted to Shengjing Hospital Affiliated to China Medical University were divided into meniscus injury group(67 cases) and without meniscus injury group(31 cases) according to whether they had meniscus injury.The patients with anterior cruciate ligament rupture and meniscal injury were divided into study group(37 cases underwent arthroscopic reconstruction of anterior cruciate ligament and meniscus injury), and 30 cases in control group(only meniscus was repaired under arthroscopy). The factors affecting the anterior cruciate ligament rupture associated with meniscus were analyzed, and the surgical outcomes of the study group and the control group(the cure rate of meniscus injury and the 1-year reoperation rate, IKDC and Lysholm knee function score) were compared. \n \n \nResults \nMultivariate logistic regression analysis showed that early course of disease[95%CI(1.444, 41.68), P<0.05], middle course of disease[95%CI(1.682, 52.147), P<0.05], chronic phase[95%CI(3.623, 180.32), P<0.05]), history of recurrent injury[95%CI(2.649, 27.222), P<0.05]) were risk factors of meniscus injury caused by rupture of anterior cruciate ligament.The treatment rate of meniscus injury in the study group[89.19%(33/37)]was higher than that in the control group[66.67%(20/30)], the reoperation rate in the study group[5.41%(2/37)]was lower than that in the control group[26.67%(8/30)], the differences were statistically significant (χ2=5.084, 5.898, all P<0.05). 摘要

目的探讨前交叉韧带断裂合并半月板损伤的相关因素及关节镜手术的效果。方法选取2014年7月~ 2018年5月中国医科大学附属盛京医院收治的前交叉韧带断裂患者98例，根据是否有半月板损伤分为半月板损伤组(67例)和无半月板损伤组(31例)。将前交叉韧带断裂及半月板损伤患者分为研究组(37例行关节镜下前交叉韧带及半月板损伤重建术)和对照组(30例仅行关节镜下半月板修复术)。分析影响半月板合并前交叉韧带断裂的因素，比较研究组与对照组的手术结果(半月板损伤治愈率、1年再手术率、IKDC和Lysholm膝关节功能评分)。结果多因素logistic回归分析显示，病程早期[95%CI(1.444, 41.68)， P<0.05]、病程中期[95%CI(1.682, 52.147)， P<0.05]、慢行期[95%CI(3.623, 180.32)， P<0.05]、复发损伤史[95%CI(2.649, 27.222)， P<0.05]是前交叉韧带断裂致半月板损伤的危险因素。研究组半月板损伤治愈率[89.19%(33/37)]高于对照组[66.67%(20/30)]，再手术率[5.41%(2/37)]低于对照组[26.67%(8/30)]，差异均有统计学意义(χ2=5.084、5.898，均P<0.05)。术后12个月，研究组患者IKDC、Lysholm评分分别为(90.25±14.67)分、(88.36±11.25)分，显著高于对照组[(73.52±10.12)分、(71.47±10.68)分](t=12.129、19.309,P均<0.05)。结论前交叉韧带断裂合并半月板患者与既往及复发损伤史有关。关节镜下手术同时重建前交叉韧带和半月板损伤可显著改善膝关节功能，提高该类患者半月板损伤的治愈率，降低短期再手术率。关键词:前交叉韧带;半月板,胫骨;膝关节;伤口和伤害;关节镜;因子分析、统计学
