Our hybrid telehealth modality flow board summarizes how patients received care from volunteers in person and remotely. Successes of the modality included collecting medical records before the visit, creating more defined roles, and implementing e-prescriptions. Challenges faced included recording vital signs and communicating between volunteers on-site and remotely.
