Preservation of the humeral head is always considered except in cases with severe comminution and compromised vascularity where shoulder hemiarthroplasty is an alternative option.

Case report: We present two male patients, 57- and 62-years-old who sustained such a complex proximal humeral fracture after a high-energy injury.
