Unusual cytology result were 862 cases, with 356 ASCUS, 84 ASCH, 216 LSIL, 184HSIL and 22 cancer. The number of biopsy > or = CINI/HPVI was 682, positive rate of HR-HPV was 78.59% (536/682). In screening CIN II or above, sensitivity, specificity, PPV and NPV of TCT were 88.94%, 32.73%, 48.49%, 80.60%, of HR-HPV DNA detectiort by HC-II were 90.21%, 51.82%, 57.14%, 88.14%, and of HR-HPV detection combined with cytology were 97.45%, 22.42%, 47.22%, 92.50%.</p><p><strong>Conclusion: </strong>The infection rate of HR-HPV in cervical lesions is higher in each age group. Infection rate of HR-HPV is ascending with serious degree of cervical lesion. HR-HPV detection by HC- II is an important method in screening cervical lesion. 摘要

目的:分析宫颈病变妇女高危人乳头瘤病毒(HR-HPV)的感染情况,评价混合捕获ⅱ型(HV-II)检测高危人乳头瘤病毒在宫颈病变特别是高级别宫颈上皮内瘤变(CIN)筛查和诊断中的意义。方法:2009年6月至2008年12月,对1130例宫颈病变患者进行宫颈细胞学检查、HC-II HR-HPV检测、阴道镜检查及阴道镜下活检,初步诊断为宫颈病变,其中CIN及(或)尖锐湿疣(CIN /HPV I) 212例,CIN /III型442例,浸润性宫颈癌28例。宫颈细胞学检查采用薄准备液基细胞学检查(TCT), HR-HPV检测采用HC-II。结果:1130例中HR-HPV阳性占65.84%(744/1130)。细胞学异常862例,其中ASCUS 356例,ASCH 84例,LSIL 216例,hsil 184例,癌22例。活检>或= CINI/ hpv者682例,HR-HPV阳性率为78.59%(536/682)。筛查CIN II及以上时,TCT的敏感性、特异性、PPV、NPV分别为88.94%、32.73%、48.49%、80.60%,HC-II检测HR-HPV DNA的敏感性、特异性、PPV、NPV分别为90.21%、51.82%、57.14%、88.14%,结合细胞学检测HR-HPV的敏感性、特异性、PPV、NPV分别为97.45%、22.42%、47.22%、92.50%。结论:各年龄组宫颈病变中HR-HPV感染率较高。随着宫颈病变程度的加重,HR-HPV感染率呈上升趋势。HC- II检测HR-HPV是宫颈病变筛查的重要方法。HR-HPV检测具有较高的灵敏度和NPV,是治疗TCT ASCUS和LSIL妇女的可行选择。
