The conceptual framework provides an introduction to a report of a Dutch example. First of all, the social geography is described, then the parties in the health care field and the consultants' participation. Subsequently, an extensive analysis is offered of some salient observations during the process of organizing a regional federation. They are the ways the federation was founded and developed, and the problems concerning a policy program.
