Evaluación de resultados y recaídas en cirugía micrográfica de Mohs para carcinomas queratinocíticos de cabeza y cuello
Background
Keratinocitic carcinomas in photoexposed areas incidence rates are continuing to increase, and represent a challenge to achieve oncological safe margins and, in some cases, the plastic reconstructions with good esthetic and functional results, especially on the head and neck region. The aim of this article is to describe the surgical and follow-up outcomes of patients with head and neck keratinocitic carcinomas treated with Mohs micrographic surgery (MMS).
Methods
We included a consecutive series of patients undergoing MMS between 2014 and 2022, with carcinomas of the head and neck and complete pathology and follow-up registries.
Results
92 patients with 123 lesions were included; 91.8% with basal cell carcinoma. A hundred and eight lesions were located on the face, of which 62% were on the high-risk area (H). In 16.4% cases the margins of the first ring were positive on the frozen section and a subsequent excision was performed; in two patients the margins were positive on the final pathology result. During follow-up 5 recurrences were detected in four patients; they were all treated with MMS.
Conclusion
In our series patients treated with MMS for primary and recurrent keratinocitic skin cancers of the head and neck had excellent oncological outcomes.
