A 78-year-old woman was admitted with right lower quadrant abdominal pain and deterioration. Initial imaging revealed a right-sided colonic malignancy with hepatic metastases. Follow-up scans showed gas within metastases and minimal pneumoperitoneum, suggesting bowel perforation.
