Multivariate logistic regression analysis identified several significant factors associated with home treatment implementation, including child’s age, ethnicity, hemophilia subtype, assistance from medically trained relatives or friends, use of emicizumab, operator confidence in venipuncture, prior self-injection training, understanding of aseptic techniques, and availability of syringes and bandages.</p><h3>Conclusion</h3><p>The implementation of home treatment for children with hemophilia requires further improvement. Particular attention should be given to younger patients. 目的:了解中国血友病儿童家庭治疗的现状,找出影响儿童血友病家庭治疗的关键因素。方法:在全面查阅文献和专家咨询的基础上,编制《血友病儿童家庭治疗问卷》。研究对象是在“血友病之家”官方网站注册的血友病患儿的父亲或母亲。从2022年6月1日至2023年7月1日,通过短信集中传播的方式进行问卷调查。结果:共收集有效问卷293份。其中118例(40.3%)患儿在家中开始自我注射。多变量logistic回归分析确定了与家庭治疗实施相关的几个重要因素,包括儿童的年龄、种族、血友病类型、接受过医学培训的亲属或朋友的帮助、emicizumab的使用、操作人员对静脉穿刺的信心、先前的自我注射培训、对无菌技术的理解以及注射器和绷带的可用性。结论:血友病患儿家庭治疗的实施有待进一步完善。应特别注意年轻患者。此外,必须加强自我注射培训,增强护理人员进行静脉穿刺的信心,并提高一次性无菌用品的可得性。

Analysis of the current situation and influencing factors of home treatment for children with hemophilia in China

Objective

To investigate the current status of home treatment for children with hemophilia in China and identify key influencing factors.

Methods

Based on a comprehensive literature review and expert consultation, the Questionnaire on Home Treatment for Children with Hemophilia was developed. The study targeted fathers or mothers of children with hemophilia who were registered on the official website of “Home for Hemophilia Patients.” From June 1, 2022, to July 1, 2023, a questionnaire survey was conducted through centralized dissemination via short message service.

Results

A total of 293 valid responses were collected. Among them, 118 children (40.3%) had initiated self-injection at home. Multivariate logistic regression analysis identified several significant factors associated with home treatment implementation, including child’s age, ethnicity, hemophilia subtype, assistance from medically trained relatives or friends, use of emicizumab, operator confidence in venipuncture, prior self-injection training, understanding of aseptic techniques, and availability of syringes and bandages.

Conclusion

The implementation of home treatment for children with hemophilia requires further improvement. Particular attention should be given to younger patients. In addition, it is essential to enhance self-injection training, boost caregivers’ confidence in performing venipuncture, and improve the availability of disposable sterile supplies.
