A 65-year-old male sustained bilateral open knee dislocations in an agricultural accident. The left knee was treated with early multiligamentous reconstruction, achieving full recovery. The right knee, complicated by infection and extensive bone loss, required multiple debridements, soft tissue coverage with a medial gastrocnemius flap, and primary arthrodesis. At 18 months, the patient walked independently, with full function in the left knee and a stable, painless right knee.
