This condition is rare. We present the management of a case of giant prostate hyperplasia (541 grams on CT scan) with a surgical enucleated volume of 800 grams in a patient who presented with haematuria. An abdominal-pelvic CT scan was required to differentiate it from a suspected bladder tumour. This case was successfully managed with a favourable outcome following open transvesical prostatectomy, ensuring minimal blood loss.
