Reduced mobility, sensory loss, incontinence and cognitive impairment are some of the factors known to elevate pressure ulcer risk. Nurses have an important role to play in the assessment of pressure ulcer risk and the implementation of preventive strategies, alongside other multidisciplinary professionals. Inconsistencies in evidence-based practice can lead to avoidable harm in health care.
