This review explores these challenges in two parts: (1) practical human factor issues such as distractions, interruptions, and fatigue, and (2) cognitive biases that impact diagnostic accuracy. Evidence-based mitigation strategies, including environmental optimisation, structured reporting, and cognitive debiasing techniques, are also discussed.
