The purpose of this study was to identify the challenges and solutions for improving the stewardship of rehabilitation services in Iran in 2024.</p><p><strong>Materials and methods: </strong>This study employed a qualitative methodology using a framework analysis approach. Data were collected through in-depth semi-structured interviews. Purposive sampling with maximum variation was used. To ensure the data trustworthiness, Lincoln and Guba's criteria were applied.</p><p><strong>Results: </strong>Sixteen interviews were conducted. Key challenges included: fragmented and unclear stewardship of rehabilitation services; lack of integration of rehabilitation services into the health system; absence of sustainable funding mechanisms. Key strategies include: establishing the High Council for Rehabilitation under the supervision of the President; expanding comprehensive rehabilitation services at all health system levels; optimizing the utilization of existing capacities to enhance rehabilitation services; securing sustainable financing; and creating an information management infrastructure.</p><p><strong>Conclusions: </strong>The findings highlight two critical strategies for robust rehabilitation service stewardship, fostering coordination and integration among policymaking entities and establishing a comprehensive rehabilitation information system.
