A gradual increase in the 2-year RS rate was observed over successive chronological diagnostic periods. Age above 75 years at diagnosis (HR 3.65, 95% CI: 2.73–4.89) and diagnosis during a calendar period of 1995–2006 (HR 1.30, 95% CI: 1.10–1.53) were associated with a significantly increased risk of death.
