The patient developed a large inferolateral ventricular pseudoaneurysm, an anterolateral mitral paravalvular leak with moderate mitral regurgitation, and a complete atrioventricular block. Given the high risk associated with a third conventional open-heart surgery, a hybrid off-pump transapical approach was proposed.
