Close communication between the anaesthetic and surgical team is vital in providing adequate one-lung ventilation and a successful operation. We describe a case of intra-operative iatrogenic surgical bronchial tear and subsequent bronchial cuff rupture, requiring immediate specialist anaesthetic management.
