The patient underwent successful open surgery after adequate preoperative preparation.</p><p><strong>Clinical discussion: </strong>Zenker's diverticulum is an uncommon cause of pharyngeal dysphagia, but it should be considered in the differential diagnosis due to the importance of early intervention and prevention of malignancy.
