We describe an intraoperative intercostal cryo-neuronal pain block as one possibility for postoperative pain relief. <b>Case description</b> A 63-year-old male patient underwent minimally invasive mitral valve repair. To reduce postoperative pain, an intercostal cryo-neuronal ablation was performed.
