An emergency bilateral fasciotomy was performed, and the patient fully recovered within two months without any neuromuscular dysfunction.</p><p><strong>Conclusions: </strong>ACS should be an important differential diagnosis for lower extremity pain after gynecologic surgery, especially with prolonged lithotomy positioning.
