越来越多的证据表明,多动脉心肌血管重建术优于单动脉心肌血管重建术。多动脉移植的围手术期效果相当,但长期疗效更佳。将桡动脉(RA)作为继左胸内动脉(LITA)之后的第二条动脉移植物是实现这一目标的最佳且最简单的方法,前提是使用痉挛预防措施并避免竞争性血流。90%以上的患者都有可能使用 RA,它完全可以用作无隐静脉移植(SVG)。RA长而坚固,比右ITA更宽,易于采集和处理,用途广泛,可单独或依次从主动脉或作为Y型或延伸型移植物使用,可到达任何目标。与 LITA 同时采集非常省时。两种 RA 均可使用。RA的短期通畅率非常高,在观察研究和随机试验(RCT)中超过90%。一旦在部署时没有出现技术问题,RA 就能永远保持通畅,通常能保持完美的通畅性。15-20 年的通畅率(包括早期失败)为 87%-90%,心脏不良事件较少,在随机试验中存活率较高。
