Bilateral epididymal tuberculosis is an exceeding rare cause of infertility in young, and the GeneXpert MTB/RIF test is considered a useful tool to the diagnosis confirmation. The major routes for scrotal tuberculosis are retrograde extensions from the infections in the prostate or seminal vesicles, and pelvic hematogenous or lymphatic disseminations.
