Trichology plays a crucial role in dermatology due to the high prevalence of various hair pathologies and their impact on patients' quality of life. Among them, cicatricial alopecia constitutes a group of diseases characterized by different inflammatory conditions that cause the destruction of hair follicles and their replacement by fibrous structures. Although in recent years we have seen important advances, the management of these pathologies constitutes today a major challenge for the dermatologist. The purpose of this review is to provide an overview of primary cicatricial alopecia structured in two groups, lymphocytic, specifically frontal fibrosing alopecia, lichen planus pilaris and central centrifugal alopecia; and neutrophilic with folliculitis decalvans and dissecting cellulitis as representatives. The main novelties are presented in a practical way, highlighting the most significant advances in terms of their pathophysiology, clinical manifestations, diagnosis and therapeutic approaches for each of them.
