This article provides an overview of state-of-the-art surgical principles and techniques for achieving surgical radicality in localized pancreatic cancer, with a particular emphasis on artery-first approaches, the triangle operation, arterial and venous dissection techniques, including arterial divestment, and the significance of R0 resections with complete lymphadenectomy.
