This study aims to determine the mechanism for managing and reporting vaccine data using an electronic system at Gatoel Mojokerto Hospital in 2022. Methods: This study was a qualitative research with two medical recorders and health information as the subject. Information was collected used nonparticipatory observation methods and in-depth interviews. Results: The management and reporting of COVID-19 vaccine data at Gatoel Mojokerto Hospital integrating three systems, namely SIMRS producing daily vaccination reports for the Director of Gatoel Mojokerto Hospital, BPJS primary care vaccination system for claims for vaccine costs and vaccine procurement, and the Gayatri system. Mojokerto City for monitoring reports on the achievements of the vaccination program to the Mojokerto City Health Office. The information system for managing and reporting vaccine data at Gatoel Mojokerto Hospital applies a decentralized model.
