Results: Based on previous experiences of patient education about their medications, and through an iterative process involving all key stakeholders, a prototype of the shared treatment plan has evolved and is currently reaching production status. It is based on the e-toile secure architecture of federated documents, and is organised in a modular way to enable various interfaces with existing prescription and dispensation systems. The first version is provided as a web-interface on the e-toile portal, but other mechanisms of integration are possible and will be used to streamline processes around medication reconciliation, as well as integration on larger projects such as epSOS, the European-wide interoperability eHealth platform. Discussion: Initial feedback from key stakeholders shows a strong interest in this functionality. Its feasibility has been demonstrated on the e-toile platform.
