This valuable information can be used in the educational fields to provide constructive feedback for surgeons and other staff in the operating room (OR) environment. Specific error tracing techniques and recording tools are required to extract this information. To be able to design a sufficient tool, the first step is defining errors and understanding their descriptions. Thus, this study reviewed several articles related to errors definition and standard error categories. The next step is rating errors for future analyzing. For the error rating tool, the Objective Structured Assessment of Technical Skill (OSATS) rating scale was reviewed. Finally, in order to detect errors in the OR, tools with the ability to record intraoperative surgical information in the video and audio formats were reviewed.
