The compression of the posterior interosseous nerve can happen either before, or through or after the course of the nerve in the supinator muscle. Tumors could rarely lead to secondary compression of the nerve by mass effect. We describe a case of a lady aged 73 years with a posterior interosseous nerve syndrome due to an intermuscular lipoma. The patient had appreciable recovery after surgery and physiotherapy.
