The objective of this study was to analyze the outcomes of brain abscesses and to identify factors associated with patients’ outcomes. Methods: The characteristics (age, sex, medical history, initial neurologic status, laboratory data, abscess morphology) and outcomes of 54 patients with brain abscesses who underwent surgical treatment at our hospital from January 2011 to December 2020 were retrospectively analyzed.Results: There were more male patients than female patients (44:10). Sixteen patients died or became disabled, whereas 38 patients recovered. Statistical analyses indicated that those with a penetration cause, Glasgow coma scale≤12, ventriculitis, or Gram-negative bacilli grown in abscess culture had significantly poorer outcomes, whereas those with a contiguous cause and isolated brain abscess had significantly better outcomes (p<0.05). Conclusion: This study identified poor prognostic factors in brain abscess patients. Despite advances in diagnosis and treatment techniques, brain abscess still has a high mortality rate. 摘要

目的:脑脓肿是一种较为罕见的脓肿,因为大脑有丰富的血供和血脑屏障的保护。本研究的目的是分析脑脓肿的预后,并确定与患者预后相关的因素。方法:回顾性分析2011年1月至2020年12月在我院行手术治疗的54例脑脓肿患者的特点(年龄、性别、病史、初始神经系统状况、实验室资料、脓肿形态)及转归。结果:男性多于女性(44:10)。16名患者死亡或致残,38名患者康复。统计分析表明,穿透性病因、格拉斯哥昏迷评分≤12、脑室炎或脓肿培养中生长革兰氏阴性杆菌的预后明显较差,而连续性病因和孤立性脑脓肿的预后明显较好(p<0.05)。结论:本研究确定了脑脓肿患者预后不良的因素。尽管诊断和治疗技术有所进步,但脑脓肿的死亡率仍然很高。本研究的结果有望帮助脑脓肿患者的准确诊断和更准确的预后。
